| BAMA PUPPIES Guntersville, Alabama 35976 Health Guarantee/ Sales Contract I, Mike Little, certify that our puppies are healthy to our knowledge at the time they leave our kennels. Puppies have received their 1st shots and have been de-wormed at least twice. We guarantee the health of our puppies as long as the buyer meets the following requirements: 1. The puppy must be examined by a licensed veterinarian within 72 hours of pickup or arrival. 2. All vaccinations must be kept current and documented by a licensed veterinarian. 3. If a serious health issue is diagnosed Bama Puppies must be contacted within 48 hours and the buyer must provide our veterinarian documentation from a licensed veterinarian of the problem. HEALTH GUARANTEE: If a health issue is diagnosed during the buyer’s initial veterinarian exam we will work with the buyer to correct the problem. If the problem is serious enough to require a replacement puppy we will try to accommodate the buyer’s preference. In the event that your puppy is diagnosed with hip dysplasia within 3 months of the date of purchase we will replace it. Replacement puppies will be limited to a puppy of equal or lesser value to the original puppy and the original puppy must be returned to us at the buyer’s expense. In order to apply the health guarantee the buyer must contact us within 7 days of the date the health deficiency was discovered and provide written documentation from two licensed veterinarians of the health issue. All veterinary charges will be the buyer’s responsibility and absolutely no refunds will be given. Unless otherwise agreed upon prior to any procedure, all vet bills for corrective surgery, cosmetic surgery, shots and normal health care, euthanasia, spay/neuter and shipping are the responsibility of the buyer. A non-refundable $100.00 deposit will be required to hold a pup. The deposit will be subtracted from the final cost of the puppy. Shipping is available at the buyer’s expense. Final payment must be received by the time the puppy is six weeks old. The payment must be in the form of a postal money order or cash if the buyer is picking up the puppy at our kennel. The buyer also agrees not to hold the breeder responsible for any actions taken by the puppy off the premises of the breeding kennel. The breeder will not be responsible for any damage, acts of aggression or injury to another person or animal caused by the dog. The buyer understands that upon taking possession of the puppy, that the buyer assumes full and complete responsibility and liability for the puppy. Note: This contract will be considered null and void if all the requirements listed above are not met and all documentation provided. I have read the above contract thoroughly and understand and agree to the terms set forth in this document.
